HealthPathways is web-based information portal supporting primary care clinicians to plan patient care through primary, community and secondary health care systems within Canterbury. It is like a 'care map', so that all members of a health care team – whether they work in a hospital or the community – can be on the same page when it comes to looking after a particular person.
HealthPathways are designed to be used at the point of care, primarily for General Practitioners but is also available to Hospital Specialists, Nurses, Allied Health and other Health Professionals within Canterbury.
These pathways, for the care and management of patients within the Canterbury, have been developed jointly by primary and secondary care clinicians.
They provide guidance particularly for general practice teams but are read by other registered health professionals to diagnose and manage patients suffering from a number of different conditions, and contain patient information resources. They are not for access or use by non-registered health professionals or patients.
The pathways are maps of publicly-funded services accessed by referral from the community, and are strongly evidence based, but are not full clinical guidelines.
As the pathways are suggested guidance only, while using them you must exercise your own clinical judgement and pertinent clinical data when treating your patient.
